This form should be filled out in May.

Dissertation

your dissertation may be rejected by the library and
you will not be awarded your degree.

Fill out the forms online and submit them to the Graduate Administrator,
004 Fisher Hall, FIFTEEN DAYS prior to your defense date. Handwritten
forms will not be accepted by the Graduate School.
Defense Checklist: NEW For
guidelines on the requirements on the Final Public Oral Exam (defense).
Advanced Degree Application: This form must be filled out and submitted electronically for a Master's
degree (after Generals requirements have been met) or for a Ph.D.
when all degree requirements have been met.
Request to Hold a Final Public Oral Form: Students must fill out this
form when they are ready to defend.
Exit Questionnaire: Students must fill this out for the Graduate School
when they defend.
